Your Records and Narrative

All patients should provide your records and narrative: Gather lab work, results, and imaging reports from the last 18 months. Request these from your doctor’s office. Copying your records can be a low priority in a busy office, so be polite – but persistent. You may be required to pay for copies. If charges seem extreme, contact us for help.

If you have had prior surgery: For each abdominal or pelvic surgery, gather the operative report, the pathology report (if any), and photos from the surgery. You can get these items from the surgeon’s office or from the Medical Records department at the hospital.

If you have never had surgery: You are still eligible for a records review; you’ll just have fewer records to review!

Narrative

Tell us your story. Describe your experience with pelvic pain and/or infertility, what has been done previously, how it has impacted your life, your expectations and even your disappointments with prior care and treatments. Tell us your goals (pregnancy, pain relief, etc.) This is very important as it will allow us to understand you better and personalize your care for you.  Your narrative can be as short as a paragraph or as long as you want it to be.

What to Send and Where to Send it

Send these items:

  • Medical records as listed above
  • Personal Narrative
  • Copy of your insurance card (both sides)
  • CRRS New Patient Packet

When your packet is complete, send it to us. Mail it (keep a copy) to the office at: CRRS

Please do not send partial materials. We have no way of knowing if anything is missing. If you are missing an item that’s proving difficult to get, let us know as soon as you can and we’ll try to help you. We’ll contact you to let you know we have your packet, and Dr. K will call you within a week of that time.
